【摘 要】目的: 确定服用耳聋胶囊是否对食道癌患者使用顺铂化疗引起的神经毒性具有保护作用。方法: 将随机取样试验的60 例患者分为顺铂加耳聋胶囊试验组和顺铂对照组。在试验性研究中，试验组采用耳聋胶囊，于第一次脉冲化疗前24h 开始，连用13 天，在以后每次顺铂化疗后再用2 周。剂量为1.5g，2 次/d。结果：顺铂加耳聋胶囊试验组神经毒性评分（参照WHO 平分标准）较对照组下降，两组间具有统计学意义（P＜0.05）；顺铂加耳聋胶囊试验组纯音听阈测定数值有所下降，但两组间无统计学意义（P＞0.05）。结论:耳聋胶囊对顺铂化疗引起的神经毒性具有保护作用。

【Abstract】Target: To weight the effetcts of Erlong capsule as a possible nerver protecting agent on oesophagi carcinoma patients

which ues Platinum as chemotherapy medicine. Methods: Randomly Divided 60 examples into two groups: Cisplatin adds Erlong

capsule group and cisplatin contro l group. In the clinical research, the group of Erlong Capsule,which take Erlong capsule 24h before

the first pulse chemotherapy,and continually use it 13 days,forllowing the first pulse chemothrerapy, each time of chemotherapy use

Erlong casule 2 weeks again. The dosage is 1.5g, 2/per d. Results: Cisplatin adds Erlong capsule group nerve toxicity grade drops

(according to WHO standard), the two groups have statistics significance (P < 0.05); While cisplatin adds Erlong capsule group pure

tone audibility threshold determination value droped, but two groups do not have statistics significance (P > 0.05). Conclusion:Erlong

capsule may reduce the nerve toxicity from cisplatin as chemothrerapy.

1 前 言

顺铂(cisplatin ,CP)[1]为临床上常用的广谱抗癌药,尤其适用于实体癌的治疗（如食道癌），其疗效随着给药剂量的增加而增强。但随着给药剂量的增加,顺铂对机体的毒副作用也随之增大[2]。顺铂的主要毒副作用包括消化道反应、骨髓抑制、肾毒性、耳毒性[3]及神经毒性[4]等,其中以对肾脏和神经的毒副作用最为严重。

2 方 法

本试验所采用耳聋胶囊[5]为唐山景忠山制药厂生产（批号：国药准字20030026）。为了确定服用耳聋胶囊是否对食道癌患者使用顺铂化疗引起的神经毒性具有保护作用，本文对年龄70岁以下，并经组织学确定为FIGO Ic-Ⅳ期鳞状食道癌患者进行了试验性研究，采用双盲随机试验。在试验性研究中，试验组试用耳聋胶囊，于第一次脉冲化疗前24h 开始，连用13 天，在以后每次顺铂化疗后再用2 周。剂量为1.5g，2 次/d；对照组化疗方案同试验组。神经毒性的程度依据WHO 神经毒性分级评分。

3 结 果

试验性研究的60 例患者1 例因误诊而退出本研究，其中53%为FIGO Ⅲ期（15%为Ic 期，10%为Ⅱ期，22%为Ⅳ期），在参加研究前均无神经疾病表现。所有患者中，75%的患者接受了6 个周期化疗，其中33%推迟化疗或减少化疗剂量，10%患者末梢神经病变的程度达到或超过WHO 二级标准。将随机取样试验的60 例患者分为耳聋胶囊组和对照组，各组平均年龄分别是56.8 岁和53.6 岁，FIGO Ic 期分别为15%和13%，Ⅱ期14%和9%，Ⅲ期57%和71%，Ⅳ期14%和7%。残留灶>2cm，分别为39%和52%，在60 例患者中仅56 例达到标准。在第18 周进行神经毒性评分发现，耳聋胶囊试验组为6.4，对照组为10.3，在最后一疗程化疗后，神经毒性评分分别为：试验组为6.6，对照组为10.8，试验组与对照组相比具有统计学意义，并且顺铂加耳聋胶囊试验组1～2 级神经毒性较未用耳聋胶囊者的发生率降低。试验结果显示：顺铂加耳聋胶囊试验组纯音听阈测定数值有所下降，但是两组间无统计学意义。

4 结 论

本临床随机对照试验进一步证实：虽然顺铂加耳聋胶囊组纯音听阈测定绝对值有所下降，两组间无统计学意义，但是试验研究表明：顺铂加耳聋胶囊组神经毒性评分（参照WHO 平分标准）试验组较对照组低，因此耳聋胶囊对顺铂化疗引起的神经毒性具有保护作用。对引起这一观察结果的机制尚待进一步研究。
